Philadelphia has carried one of the highest overdose death rates of any large American city for most of the past decade. The reason is chemical as much as social: the local opioid supply is almost entirely fentanyl, and it is routinely cut with xylazine — a veterinary sedative that lengthens sedation, causes severe skin wounds, and does not respond to naloxone the way an opioid does.
