Small-Group Therapy for Addiction in Pennsylvania

Our process groups run with five to seven people at Phoenixville, Center City Philadelphia and Avondale, and in virtual IOP throughout Pennsylvania.

Small caseloads are a clinical decision, not a marketing line: in a group of six you get talked with. In a group of twenty you get talked at.

Why group size decides everything

Group therapy is where people discover that their thinking is not unique, their shame is not accurate, and other people have already survived the week they are dreading.

But that only happens if there is airtime. A ninety-minute group with twenty people gives each person four minutes. With six people, you actually get worked with — every session.

Our groups are facilitated by licensed clinicians, not run as open-ended sharing circles. Each group has a clinical focus, and your individual therapist knows what came up in it.

The same small caseload means staff notice when you go quiet, when you miss a day, when something has changed. Anonymity is not a feature of good treatment.

The groups in a typical week

PHP runs about six hours a day, five days a week; IOP runs nine to fifteen hours a week in day, evening or virtual tracks.

Process group

The core group: what is actually happening in your life this week, worked through with clinician facilitation and peer feedback.

CBT skills group

Trigger mapping, thought records and relapse prevention planning done together, then practiced at home.

DBT skills group

Distress tolerance, emotion regulation, mindfulness and interpersonal effectiveness, taught in rotation.

Seeking Safety group

Present-focused work for clients carrying PTSD alongside substance use — no trauma retelling required.

Recovery pathways group

12-step, SMART Recovery and secular approaches presented as options. You choose your path; we do not assign one.

Family and gender-specific groups

Space for the issues that only get honest in the right room — including sessions with partners, parents and adult children.

Who small groups help most

Almost everyone benefits from group work, but small groups matter most for the people who get lost in large rooms.

  • Anyone who has sat through a large group program and never once spoken.
  • Clients who are isolated — working from home, newly single, new to the area, or estranged from family.
  • People with social anxiety who need a room small enough to be survivable.
  • Professionals who want peers who understand the pressure they are under.
  • Clients stepping down from PHP to IOP who want to keep the same peer group.

The evidence behind group treatment

SAMHSA's treatment improvement protocol on group therapy describes it as a core modality in substance use treatment, with cohesion and active participation among the factors that predict benefit.[1]

Participation — not attendance — is the mechanism. That is the entire argument for capping group size.[1]

NIDA's principles also emphasize that treatment must be tailored to the individual and reviewed continually, which is only realistic when clinicians carry small caseloads.[2]

Our completion rate of 53%, against a national outpatient average near 26%, is the clearest evidence we have that people stay when they are known by name.

How many people are in a group?

Five to seven. That cap is why every client gets meaningful time in every session.

Do I have to talk in group?

Not on day one. Most people listen first. Within a week or two, almost everyone talks — a small room makes that far easier.

Is group confidential?

What is said in group stays in group; every member agrees to it as a condition of participation, and clinicians reinforce it.

Is it all 12-step based?

No. We present 12-step, SMART Recovery and secular pathways and support whichever you choose.

Can I attend groups virtually?

Yes. Virtual IOP groups are held by secure video for clinically appropriate clients anywhere in Pennsylvania.
