Marijuana in Pennsylvania: legal does not mean harmless

Cannabis use disorder is one of the most common substance use disorders in the country, and most people who have it never label it addiction because the drug is legal somewhere nearby.[1][3]

Daily high-potency use is associated with a substantially higher risk of psychosis, and heavy adolescent use is linked to lasting effects on attention, memory and learning.[1][2]

In Chester and Montgomery counties, we see high-functioning clients — students at West Chester and Penn State satellites, trades workers, KOP office staff — who are not in trouble with the law but cannot get through a day without a cart or a dab pen.

In Philadelphia, the bigger issue is what the weed is with: cannabis plus alcohol, benzos or unregulated street product sold as "loud." Polysubstance use is the norm, not the exception.[3]

Marijuana addiction treatment programs we offer

There is no FDA-approved medication for cannabis use disorder, so treatment is therapy-driven — and that is exactly what our clinical day is built for.

Cognitive behavioral therapy (CBT)

CBT is the best-supported treatment for cannabis use disorder. It targets the automatic loop — boredom, anxiety, sleep, pen in hand — and replaces it with skills you actually practice between sessions.

PHP & IOP levels of care

PHP runs about six hours a day, five days a week; IOP runs nine to fifteen hours a week with day, evening and virtual tracks. Both let you live at home while you rebuild a life that does not revolve around being high.

Dual diagnosis & psychiatry

Most heavy cannabis use we treat is medicating anxiety, ADHD, insomnia or trauma. Psychiatric evaluation and medication management happen inside the program so the underlying condition is treated too.

Motivational enhancement

Ambivalence is normal when a drug is legal and your friends all use it. Motivational interviewing works with that instead of arguing you into a decision you have not made yet.

Contingency management & structure

Small caseload groups of five to seven, two individual sessions a week, and a schedule that fills the hours you would otherwise be smoking through. Structure does a lot of the work early on.

Holistic & experiential therapy

Yoga, meditation, Reiki, soundbath, art and recreational therapy, a full on-site gym, basketball and monthly dietitian counseling — because sleep, appetite and mood all take a hit in the first few weeks without THC.

Cannabis withdrawal is real — and it is what pulls people back

Cannabis withdrawal syndrome is recognized in the DSM-5 and typically starts within a day or two: irritability, anxiety, vivid dreams, night sweats, appetite loss, and insomnia that can last a week or more.[1]

Cannabinoid hyperemesis syndrome — cycles of severe vomiting relieved only by hot showers — is increasingly common in daily concentrate users and often gets misdiagnosed for years.[2]

Withdrawal is not dangerous the way alcohol or benzodiazepine withdrawal is, but it is miserable enough that most people quit quitting on day three. Being in a program during that week changes the odds substantially.

Can you really be addicted to weed?

Yes. Cannabis use disorder is a clinical diagnosis, and about 3 in 10 people who use cannabis meet criteria for it. Legal status has nothing to do with whether a drug is habit-forming.[1]

I have a medical card — is this still a problem?

It can be. A card makes access legal; it does not make daily high-potency use harmless. If dosing has climbed, you cannot take a day off, or you are using to function rather than to treat a symptom, an assessment is worth an hour of your time.

How long does marijuana withdrawal last?

Symptoms usually begin within 24 to 72 hours, peak in the first week and fade over two to four weeks. Sleep and vivid dreams are typically the last to settle.[1]

Do I need detox for marijuana?

Almost never. Cannabis withdrawal is uncomfortable but not medically dangerous, so most clients start directly in PHP or IOP. If alcohol, benzodiazepines or opioids are also in the picture, we arrange detox first.

My teenager or young adult is smoking constantly. Can you help?
